Or if you can provide a letter from a physician stating you have a permanent disability.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p><strong>To submit a Kid\u2019s Club Cancellation request:&nbsp;<\/strong><\/p>\n\n\n\n<p>Email us at&nbsp;<a href=\"mailto:fitness@goldsgym1965.com\">fitness@goldsgym1965.com<\/a>.<\/p>\n\n\n\n<p>Include your membership agreement number.
